Job Description:

A company is looking for a Licensed Practical Nurse Care Navigator.
Key Responsibilities

Engage in monthly telephonic patient interactions to build relationships and support care goals
Create individualized care plans and manage coordination via proprietary software
Document all interactions and escalate clinical concerns as necessary

Required Qualifications

Active Licensed Practical Nurse or Licensed Vocational Nurse license
Minimum 2 years' clinical experience
Experience working with EMR systems
Familiarity with chronic condition management and individualized care planning
Intermediate to advanced computer skills
