Evolve Healthcare Inc. is seeking a Social Worker for an immediate opportunity supporting a patients in Long Beach area medical center. The selected candidate will provide 8 hour day shift support for a minimum of 6 weeks.
Qualifications/Requirements:

• Minimum of one to two years hospital discharge planning experience.
• Valid CA LCSW/LSW
• Masters of Social Work required
• Valid AHA BLS card
• Covid vaccination with booster or notarized exemption
• Experience in working with a diverse population with a wide range of problems (i.e., death and dying, maternal and child care, substance abuse, mental health, disability, chronic illness, etc.).
  Compensation: Negotiable
